Most Shopify stores do not need a fully custom theme on day one. The better question is what actually requires custom work.

Start with the theme you already have
Modern Online Store 2.0 themes expose sections and blocks merchants can rearrange. Before writing custom Liquid, check whether the layout fits an existing section, whether an app block is enough, and whether metafields can surface the data you need.
When custom sections make sense
Custom sections are worth it when merchandising logic is unique to your catalog, for example a comparison table driven by product metafields, or a flow that must stay in sync with variant inventory.
Use theme settings for presentation, apps for common features, and custom sections only where they create clear merchandising or conversion value.
